The Practice Transformation Manager partners with provider groups and internal regional leaders to drive risk capture, Quality (STARS), and Value Based Care (VBC) performance for health plan clients across assigned market. This role reports to the Director of Practice Transformation and is responsible for translating data into action, enabling Allymar point of tool adoption, and sustaining operational excellence in risk adjustment and quality outcomes.
Requisitos:
  • Experience training providers and network teams on VBC reports and related tools; comfort enabling practice level technology adoption.
  • Certified Professional Coder (CPC), CRC, RN, or other clinical/coding credential.
  • Knowledge of care gap closure strategies, documentation improvement (CDI), and pre-visit planning/point-of-care workflows.
  • Experience supporting multi-state provider networks and scaling best practices across markets.
  • Bilingual proficiency in English and Spanish required (written and verbal)
